We all know that providing heart failure care for the large number of patients with HF is complex, requiring close interaction and co-ordination between multiple healthcare organisations: it is all too easy for the patient to fall between the gaps. We’re delighted to have the president of the American College of Cardiology, Cathie Biga address the issues from a US perspective. Prof Theresa McDonagh will discuss how guidelines are assembled (perhaps, like the making of sausages, something we don’t want to watch too closely), and speakers from Wales and Scotland will discuss new initiatives in their countries for delivering HF care.

“What do they know of cricket who only cricket know?” The annual conference is a time for learning and updating, but some of the challenges facing us in our professional lives have become increasing prominent over recent years. We’re delighted to host a session featuring some of the issues. Dr Peter Wilmshurst has a long history of being unafraid in the face of power and will talk about his experiences in relation to whistle-blowing; Professor Sue Carr, Deputy Medical Director of the GMC, will give an update on GMP, training and work towards regulation of PAs; and Dr Adrian Plunkett will talk about learning from excellence: “human performance can be improved through the use of positive feedback: this approach is underutilised in healthcare” – and so say all of us!

Value-based healthcare is the equitable, sustainable and transparent use of the available resources to achieve better outcomes and experiences for every person. Straightforward in principle, but difficult to embed in practice. It can mean different things to different people and to different health care systems. We welcome Dr Mike Valentine and Dr Victor Soukoulis from the Virginia Chapter of the ACC to discuss the issues in relation to US health care, and the President of the Royal College of Physicians, Dr Sarah Clarke, to discuss the Principles of quality improvement through GIRFT.
